Golden Heart Hospice was created with one purpose: to support patients and families in our community during life’s most delicate moments. From our office in Schiller Park, our team brings expert, heart-centered end-of-life care directly to wherever you call home.
We believe no one should face a life-limiting illness in pain, in fear, or alone. Our nurses, aides, social workers, chaplains, and volunteers work as one team, focused on comfort, dignity, and the wishes of each patient and family we serve.

One Patient, One Dedicated Team
Hospice is team care. Every Golden Heart patient is supported by an interdisciplinary group that meets regularly to review comfort, symptoms, and family needs.
Hospice Physicians
Oversee the plan of care, manage complex symptoms, and coordinate with your own doctor.
Registered Nurses
Visit regularly, manage pain and medications, teach families, and are on call 24/7.
Aides & Volunteers
Help with personal care and daily comfort, and offer companionship and respite.
Social Workers & Chaplains
Guide families through decisions and paperwork, and provide spiritual and grief support.
